Study Comparing PROMs of Anterior vs. Posterolateral Approaches in Total Hip Arthroplasty Receives AAHKS Clinical Research Award

Dallas, November 9, 2019 — A study generating considerable discussion at the 2019 AAHKS Annual Meeting found no evidence for higher patient reported outcome measures (PROM) scores from patients who received total hip arthroplasty (THA) using the direct anterior approach (DAA).

Nicholas Sauder, BA, presented results from a collaboration of six U.S. centers and 398 patients whose PROMs were collected at follow-up visits between 1.5 months and 5 years, using literature-defined patient acceptable symptom state (PASS) and minimal clinically important improvements (MCII) measures.

According to the study, “the DAA patients were significantly less likely to reach the Harris Hip Score PASS at the postoperative visit than the posterolateral patients, but not at later visits. The DAA patients had no difference from the posterolateral patients in their tendency to reach the Pain MCII at the postoperative or 1-year mark. The DAA patients were less likely to reach the Function MCII at the postoperative visit, but not at the 1-year visit.”

Mr. Sauder and co-authors, Veronique Vestergaard, MD, Saira Siddiqui, MPH, Vincent P. Galea, PhD, Charles R. Bragdon, PhD, Henrik Malchau, MD, PhD, Karim A. Elsharkawy, MD, James I. Huddleston, MD and Roger H. Emerson, MD wrote that, “the results did not support faster THA recovery after DAA, and instead showed limited evidence that the posterolateral approach may result in faster recovery due to greater hip function.” In their presentation, they concluded that “no difference in any PASS or MCII threshold was found at 1, 3 or 5 years – indicating any differences in function or pain may be short-lived, and that patients reach similar final symptom states regardless of approach.”
